
Keratoconus is a progressive eye condition that causes the cornea to become thinner and develop a cone-like shape. As the cornea changes, vision can become increasingly blurred or distorted. Although keratoconus is often described in four stages, an eye doctor must evaluate several measurements to determine its severity.
Under the traditional Amsler-Krumeich classification system, Stage 4 is considered the most severe form of keratoconus. Stage 3 is generally considered advanced and may also cause significant vision problems.
Stage 4 keratoconus is commonly associated with extreme corneal steepening, substantial thinning, central corneal scarring, and a prescription that is difficult to measure accurately. However, classification systems differ, and a stage number does not always reflect how strongly keratoconus affects a person’s daily vision.
Modern systems such as the Belin ABCD grading system evaluate the front and back curvature of the cornea, its thinnest point, and corrected visual acuity. This provides a more detailed assessment than relying on one measurement alone.
As keratoconus progresses, glasses and standard soft contact lenses often become less effective. Symptoms and clinical findings associated with advanced or severe keratoconus may include:
A sudden decline in vision, eye discomfort, or cloudy appearance should be evaluated promptly. Advanced keratoconus can occasionally lead to acute corneal hydrops, which causes sudden swelling and clouding of the cornea.
A comprehensive keratoconus evaluation goes beyond a standard vision test. Corneal topography or tomography creates detailed maps of the cornea, helping an eye doctor assess its curvature, thickness, symmetry, and changes over time.
Regular monitoring is important because keratoconus may progress even before the patient notices a major difference in vision. Comparing scans from multiple visits helps determine whether the condition is stable or worsening.
Glasses may provide adequate correction during the early stages of keratoconus. As the cornea becomes more irregular, specialty contact lenses are often needed to create a smoother optical surface.
Rigid gas permeable, hybrid, and scleral lenses may provide sharper and more stable vision for patients with moderate to severe keratoconus. Scleral lenses vault over the irregular cornea and rest on the white portion of the eye, which often improves both clarity and comfort.
